$11,250 a month can disappear from a small practice without a single insurance cut, ad change, or “slow season.” The leak is simpler and more painful: inbound phone calls that go unanswered when the front desk is slammed, it is after hours, or the team is juggling check-ins, insurance calls, and ringing lines.

We walk through the exact math behind missed calls, using a conservative baseline of 150 missed calls per month, a 25% new patient conversion, and a $300 average new patient visit value. Then we dig into the real-world behavior that makes this problem worse: most callers who hit voicemail never leave a message, and many immediately book with the next practice that picks up. That turns “we will call you back” into a quiet pipeline for your competitors.

Next, we demonstrate a voice AI answering service built for medical practices that can pick up 24/7, hold a natural conversation, send a text with your online booking link, answer common questions like insurance and parking, and transfer calls to your team during business hours with context attached. We also address HIPAA concerns, why the phone layer can be a safer place to start than AI that touches patient charts, and what setup typically looks like when implemented correctly.

If you want more booked appointments, less front-desk chaos, and a better first impression for patients, start by fixing the phone. Subscribe, share this with a practice owner who needs it, and leave a review so more clinicians can stop losing patients to voicemail.

The $11,000 Monthly Leak

SPEAKER_00

Hey guys, here's a number that should bother you. $11,000. So that is roughly what the average small practice loses every single month. And the most owners have no idea it is even happening. It's not insurance cuts, it's not your marketing. It is the phone calls coming in right now that nobody is answering. I'm Dr. TJ An, I'm a board-certified podiatric surgeon. I still run my own practice, and I also help doctors across every specialty keep more of what they earn. And when I finally ran this math on my own front desk based on the stats, it stopped me cold. So let me show you exactly where that money goes and what I did about it. Let's get into it. Okay, 150 Mr. Calls a month is costing you $11,000. Well, to be exact, $11,250. So listen, I'm not going to give you a theory here. This is the real math and it works the same whether you are in podiatry, dentistry, orthopedics, dermatology, well any specialty with a phone. The average small private practice misses about 150 calls a month. And that is a conservative number.

The Simple Math Behind Missed Calls

SPEAKER_00

For a lot of you, it could be higher. And these are not all your calls. These are the ones that go unanswered. The ones that ring out while your front desk is buried, or could be in the evening time or weekends or holidays. Now out of those missed calls, roughly 25% would have booked as new patients if someone had picked up. The rest are existing patients, of course. But one in four was a real new patient you never got to. And the average new patient visit in our world is worth about $300. So some of you do a lot more than that, but uh $300 is a fair baseline. So if you um kind of do the math, 150 Mr. Cold times 25% is about 37 new patients walking out the door every month. Now 37 times $300, that's $11,250. So every month. And this is not hype, you know, this is not a typo, and it is not exaggeration. That is just uh math. And if you do a higher value procedure, surgeries, cash services, that number is not eleven thousand. It could be twenty, thirty thousand, or sometimes more. So here's the first question I want you to sit with. How many of those calls is your practice actually missing right now? Most owners have probably never counted, you know. What happens when no one picks up? Well, patients will not wait.

Why Voicemail Hands Patients Away

SPEAKER_00

80% of patients who call and go to voicemail, they do not leave messages. They actually search again. Meaning they're gonna look at another doctors who could be your competitors down the road, and they will book with them whoever answers the phone. And even if they leave a voicemail, the worst part, they'll never call you back. Why would they? You know, they might have already found another competitors. So actually you did not just miss a call, you handed a patient to a practice down the street. I get it, your front desk could be slammed, you know. They are maybe on another line, checking someone in, fighting with the insurance company. You are probably understaffed. That is not that is not a knock on your team. That is exactly the point. You are losing real money to a problem that has nothing to do with how good your people are. There are only so many hours and so many hands. Does that sound familiar? So here's what I put into my own practice: a voice AI system that answers the phone 24-7. Now I want to be clear about what this is, because the word AI scares a lot of doctors. This is not a robot voice telling people to press 1 for appointments. Actually,

A 24/7 Voice AI Front Desk

SPEAKER_00

this is a natural conversation. And honestly, instead of me describing, let me just show you. I'm going to pull out my phone right now and call my own clinic. And this is the real agent that answers our line. So watch. Okay, so I'm going to test out our voice AI. So I'm going to call my office number.

SPEAKER_01

This call will be recorded for quality purposes. Hello, you have reached the United Foot and Ankle Surgeons answering service. This is Jenny. How can I help you today?

SPEAKER_00

Oh, so this is an answering service?

SPEAKER_01

Yes, that's right. I'm here to assist with any questions, appointment inquiries, or messages for the quote. How can I help you today?

SPEAKER_00

Yeah, um, I'd like to book an appointment.

SPEAKER_01

Live Demo Booking By Phone

SPEAKER_00

That's correct.

SPEAKER_01

The booking link has been sent to your phone by text. Let me know if there's anything else I can assist you with.

SPEAKER_00

Yeah, does it take blue cross blue shield?

SPEAKER_01

Yes. The clinic accepts blue cross blue shield insurance. Let me know if there's anything else you'd like to know.

SPEAKER_00

Do we have a parking available there?

SPEAKER_01

Yes, the clinic has its own parking lot with plenty of spaces available for your convenience. Let me know if there's anything else you'd like to know.

SPEAKER_00

Yeah, and also um, I forgot to tell you I have an ingrown toenail and I want to make sure he treats that.

SPEAKER_01

Yes. Dr. An does treat ingrown toenails both non-surgically and with minimally invasive procedures. You're all set to book through um the link I sent earlier. Let me know if you need anything else.

SPEAKER_00

Um that that will be it. Thank you very much.

SPEAKER_01

You're very welcome, Mike. Thanks for calling and have a great day.

SPEAKER_00

So you saw it. Um, I just called this is super early morning. My clinic is not open yet, yet she never misses a phone call. You know, it could be uh evening, could it be weekends? I mean, that's the power of voice AI. Okay, that's it. Nobody on my team touched that phone. She picked up, she handled my question, and she just texted me our online booking link so I can grab a time in a few seconds. So again, if you already take appointments online, you're basically ready. She will pass that online booking link. And if a caller ever needs a real person, she can hand that call to your staff cleanly during the business hours by transferring the call with the context already there. So no starting over. Think of it as a front desk teammate that never goes to lunch, never gets overwhelmed, and never lets the phone ring out. Does that make sense, guys? And I know you might be asking about HIPAA, right? I mean, I'll give you the honest answer. The done right with the right vendor and a business associate agreement in place, the phone layer here can be fully HIPAA compliant. This is the front door of your practice and answering the call, talking, a call bank number, texting your booking link, routing to your team, that can absolutely be done compliantly. So that is a very different

HIPAA Questions And Safe AI Boundaries

SPEAKER_00

conversation from letting AI dig around inside your patient charts, which I am a lot more careful about. I have a whole separate video on that, so you should watch that. But answering the phone and getting the patient booked, that is the safest, highest return place to start. So, setup is pretty quick, not a six-month project. Typically, in our case, right, our clients, we do all the work. You just fill out the questionnaire and then we do every detail setup system instructions and we train your AI agent, all done for you. A system like this is a small fraction of what one missed week of course is costing you. The recovery pays for it many times over. I'm not telling you this as a theory. I run it in my own uh office, and right now we run it for around 20 other doctors' practices too. And here's what we are seeing. One practice was missing well, $7,500 per month. Another one, $9,000, another one $12,000. This third one has two locations and could never keep the phones covered consistently across both. One system, both offices, every call answered. They're recovering around $12,000 a month again across the two locations. So these are a real practice and real numbers. But do not take my words for it. Dr. Alex Kim in Dallas told me his team now turns at least 10 missed calls a week into booked appointments. And the part he keeps coming back to is that peace of mind. Uh Dr. Anna here in New York again said it the best part is they never miss a call or message

SPEAKER_00

anymore. And they are booking six to eight extra appointments every single week. So real doctors, real practices, real numbers. And here's what almost every one of them says to me after a couple of months. Why did I not do this sooner? But honestly, the money is only half of it. Your front desk stops drowning. They are not apologizing to a full waiting room while three lines are ringing. They get to take care of the patient standing in front of them. Your patient gets a better first impression. Your patients get a better first impression because somebody or something, which is AI agent, picks up the first time and you stop bleeding revenue you already worked so hard to earn. Money is not a dirty word. It is a tool. Capturing the patient who are already trying to reach you is not aggressive. It is just answering the phone. You're not selling anybody. You are serving the people who called you first. So, start with the phone. I believe that this is the safest, easiest, and most profitable place to begin. I put some link in the description below.
